Touchstone Life Care offers a fully digital, cloud-based advance care planning (ACP) management platform that connects stakeholders across the healthcare system to ensure plans are securely available in emergencies. The platform enables aged care staff to create digital ACPs on any device through guided conversations that capture residents' values and preferences, and plans can be shared with family, GPs and trusted contacts or accessed via a unique QR code. Touchstone aims to reduce the risks associated with paper ACPs and supports compliance with incoming aged care legislation and recommendations from the royal commission. The company says its platform addresses low uptake of up-to-date ACPs in Australia and improves end-of-life care. Founder and CEO Dr Merran Cooper has framed the platform as a tool to make ACPs more accessible and actionable for clinicians and families.
